Outreach that Works: Texts, Calls, and Home Visits

This session brought together outreach specialists, program managers, clinical quality leaders, and community health workers from across HRSA-supported and partner organizations to share practical strategies for patient engagement through texts, calls, and home visits. Participants compared the effectiveness of each outreach method for different patient groups, identified common barriers and possible solutions, and applied practical strategies to improve outreach workflow and follow-up.
